Your Dog's Creatinine Is Going Up. Should You Worry?

A creatinine result can stay inside your dog's lab reference range and still be worth watching. Here's what a rising value can mean, what can affect it, and how veterinarians put the result in context.

A Common Scenario: Creatinine Rises From 1.0 to 1.5
Last year your dog's creatinine was 1.0 mg/dL. This year it is 1.5 mg/dL. Both results fall within this example lab reference range (also called a reference interval) of 0.5 to 1.8 mg/dL, so neither result is flagged as high.
So does the increase matter if both values are still considered normal? Maybe. The increase does not mean your dog has kidney disease, but it's worth looking at in context, especially if creatinine keeps rising or other kidney-related results are changing too.
What Is Creatinine in Dogs?
Creatinine is a waste product produced during normal muscle metabolism. It enters the bloodstream and is filtered mainly by the kidneys, which is why veterinarians commonly use blood creatinine as an indirect measure of how well the kidneys are filtering waste from the blood. [1]
In general, if the kidneys filter less efficiently, creatinine can build up in the blood. But creatinine is not a kidney-disease test by itself. Muscle mass, hydration, food, exercise, medications and even laboratory methods can affect the result. [1][2][3]
Creatinine Is Still Normal, but Rising. Does That Matter?
It can, but one increase does not automatically mean something is wrong. A lab reference range shows how your dog's result compares with a population of healthy dogs. It does not necessarily show what is typical for your individual dog. That's why having several results over time can be so useful. [3]
The short answer: A reference range tells you how your dog compares with a population. A trend tells you how your dog compares with their own history. A normal result is not the same thing as an unchanged result.
Why Trends Matter Even When Creatinine Is Normal
Reference ranges are intentionally broad because they are built from a population of healthy animals. A greyhound and a small terrier, a muscular young adult and an older dog with less muscle can all fall in different parts of the same range. Your dog may consistently sit within a much narrower part of that range, their own personal baseline. [3]. IRIS gives a useful example: a dog's creatinine rises from 0.6 to 1.2 mg/dL over several years but never leaves the reference range. If dehydration or increased muscle mass does not explain the change, that upward trend may reflect a meaningful decline in how well the kidneys are filtering the blood. Veterinarians call this filtering ability glomerular filtration rate, or GFR. [3]
What that example means: It does not mean every doubling of creatinine means kidney disease or a specific amount of kidney function has been lost. It shows why what matters is whether the value keeps rising, how quickly it changes, and what the other results show.
What Else Can Make Creatinine Go Up?
A higher creatinine does not automatically mean kidney disease. Several common factors can affect the result, sometimes temporarily.
Hydration and blood flow. Dehydration or reduced blood flow to the kidneys can decrease filtration and raise creatinine, even when kidney disease is not the cause. [4]
Muscle mass. More muscular dogs may naturally produce more creatinine. Breed and body size also play a role. [1]
Food. Creatinine can rise after a meal, especially if it includes cooked meat. IRIS notes that increases have been reported in the hours after eating. [4]
Exercise. Intense exercise can temporarily affect creatinine in some dogs; IRIS describes this especially in racing greyhounds. [4]
Medications. Some medications can affect blood flow to the kidneys or kidney-related lab values. NSAIDs are a well-known example. [7]
Laboratory differences. Different laboratories may use different analyzers, testing methods and reference ranges, so results are not always directly comparable. [1]
When the Kidneys Could Be the Reason
When the factors above do not explain the change, your veterinarian may consider kidney-related causes such as early chronic kidney disease, the effects of a previous acute kidney injury, a kidney infection, or certain congenital abnormalities. There are also post-renal causes, such as a urinary tract obstruction, where the problem is not the kidney's ability to produce urine but that urine cannot leave the body normally. None of these possibilities can be confirmed from a single creatinine value. [6]
How Your Vet Looks at a Rising Creatinine
Your veterinarian is rarely asking only, "Is creatinine high?" The bigger question is what pattern the result fits. Three things are especially helpful:
1. Is It Still Going Up?
One result is a snapshot. Repeated tests under similar conditions help show whether the value returns to baseline, stays stable or keeps rising.
2. Were the Tests Done Under Similar Conditions?
Hydration, fasting, recent exercise, changes in muscle mass and laboratory methods can all affect the result. Comparing tests collected under similar conditions makes the trend easier to interpret.
3. Are Other Kidney Markers Changing Too?
Creatinine is one piece of the picture. Veterinarians also look at whether other kidney-related results are changing in the same direction.
Urine specific gravity (USG). Shows how well the kidneys are concentrating urine. Looking at urine concentration together with creatinine helps veterinarians distinguish some hydration-related changes from kidney dysfunction. [3]
SDMA. Another marker of filtration that is less influenced by muscle mass. Persistent increases in creatinine and SDMA together may strengthen concern that kidney filtration has changed. [2]
BUN. Often rises when filtration falls, but it can also be affected by hydration, diet, gastrointestinal bleeding and liver function. [2]
Urine protein:creatinine ratio (UPC) and sediment. UPC helps measure protein loss in the urine. Urine sediment can identify blood, inflammation, infection, cells or crystals that may change how the results are interpreted. [4]
Blood pressure and clinical history. Kidney disease and high blood pressure can affect each other. Symptoms, medications, weight and prior results help your veterinarian understand what a lab change means for your dog. [5]
Depending on the findings, your veterinarian may also review phosphorus, potassium, calcium, albumin, hematocrit, body condition, muscle mass and imaging.
When a Rising Creatinine Deserves More Attention
It appears on more than one test rather than only once.
The samples were collected under similar conditions.
Hydration, food intake, changes in muscle mass or exercise do not readily explain the shift.
SDMA, urine concentration or urine protein are changing too.
Your dog is drinking or urinating more, eating less, losing weight or showing other symptoms.
The increase is continuing or accelerating over time.
What If Your Dog's Creatinine Goes Above the Normal Range?
When creatinine is above the lab's reference range, your veterinarian will work to determine whether the increase reflects reduced kidney filtration and whether the cause may be pre-renal (such as dehydration or reduced blood flow), renal, or post-renal (such as urinary obstruction). [6]
IRIS staging is used after chronic kidney disease has been diagnosed in a stable patient. The 2026 IRIS guidance bases initial staging on fasting creatinine measured on at least two occasions and recommends using SDMA alongside creatinine when available. [5]
The timeline matters. A rapid rise over days or weeks can mean something very different from a slow increase over several years. This is why previous lab results can be especially valuable.
Questions to Ask Your Vet
Is this a true upward trend, or could it be a one-time change?
Were the previous results run by the same laboratory and collected under similar conditions?
Could hydration, food, muscle mass, exercise or medication have affected the result?
Should we check urine specific gravity, SDMA or a urine protein:creatinine ratio?
When would you repeat the test to tell a persistent trend from normal variation?

Can creatinine rise without kidney disease?
Yes. Hydration, muscle mass, food, exercise, medications and laboratory differences can all affect creatinine. That is why a higher result is interpreted in context rather than treated as a diagnosis by itself. [3][4]
Can a dog have kidney disease with normal creatinine?
Yes. Creatinine can stay within a population reference range even when kidney filtration has changed, especially early in disease. Trends, urine findings and other markers can provide important context. [3]
Can dehydration raise creatinine in dogs?
